New Mexico – Form RPD-41326
New Mexico Form RPD-41326 Rural Health Care Practitioner Tax Credit Claim is a fillable form. Once the fillable form is completed, it will become a part of the return and will be electronically filed and/or printed with the return.
NOTE: New Mexico requires documentation to support a claim for the Rural Health Care Practitioner Tax Credit. Eligible taxpayers must obtain a Certificate of Eligibility issued by the New Mexico Department of Health (NMDOH) and provide the required supporting documentation when claiming the credit. If you do not have the ability to attach a PDF or would rather send a hard copy of your documentation, you can still electronically file your New Mexico return.
In the New Mexico electronic filing steps, the screen titled New Mexico E-File Step – Attachments will direct you to mail any supporting schedules or statements. The documents should be mailed to the New Mexico Taxation and Revenue Department after the return has been accepted electronically. This enables you to electronically file the return and submit the required documentation to the State of New Mexico.
