Ohio – School District and Local Withholding on W-2

If the school district tax withholding was not listed separately from the Ohio locality withholding on your Form W-2, enter the amounts in the Q&A interview as if they were listed separately to get the correct school district withholding amount to transfer to Ohio Form SD 100, School District Income Tax Return.

Follow the steps listed in Form W-2 – Adding, Reviewing, or Deleting to begin reporting your Form W-2, Wage and Tax Statement in the TaxAct program:

  1. In Box 15, choose OH – Ohio from the drop-down menu and enter the State ID Number.
  2. Enter the reported amounts in boxes 16-19.
  3. In the box 20 drop-list, select Ohio Locality.
  4. Click Continue and finish entering your Form W-2.
  5. Click State, then click Ohio.
  6. Click School District > Tax Return.
  7. Click Edit beside each W-2.
  8. Select the correct entry in the School District Code drop-list, then click Continue.

    NOTES

    • If the school district code is not listed on Form W-2, use The Finder website to lookup a school district by entering an address.
    • If it is a non-taxing school district, choose None selected in the School District Code drop-list and enter the city or locality name in the Locality Name box instead.
    • You should only include school district taxes from Box 19 that match the district code in Box 20.
    • If Box 20 shows a name or starts with “R” and a number, that is a city tax not school district tax. Enter the city name in the Locality Name box instead of making a selection in the School District Code drop-list.
